Situation: Thalaivi (heroine) is depressed and sick without Thalaivan (Hero) who has left her temporarily in search of wealth. She utter this poem to her friend. This poem is erotic in nature and is one of a celebrated Sangam poem [also features in a A.R.Rahman musical].
குறுந்தொகை 27, வெள்ளிவீதியார், பாலைத் திணை – தலைவி தோழியிடம் சொன்னது
கன்றும் உண்ணாது, கலத்தினும் படாது,
நல் ஆன் தீம்பால் நிலத்து உக்காஅங்கு,
எனக்கும் ஆகாது, என் ஐக்கும் உதவாது,
பசலை உணீஇயர் வேண்டும்
திதலை அல்குல் என் மாமைக் கவினே. 5

Like the Sweet milk
from the fine cow,
which has neither been suckled by the calf
nor collected in the vessel,
Lying wasted on the ground,
my pubic mons with beautiful yellow spots and
my beautiful dark skin
Is of no use to me or to my lover,
as the love sickness desires to seize them!

This poem is an openly erotic in nature
Thalaivi talks about wasting of her dark beauty [ depression and lack of eating causes skin to pale ] and her vagina – Alkul [Thalaivan is away and she cant have sexual union with him]
Alkul – straight meaning is pubic mons, vaginal area. This is the primary meaning used in Sangam literature. Women’s beauty is described by her breast and vaginal areas in these poems. Avoiding recency bias (of victorian era costumes), being topless or naked were common due to weather in Ancient India and Tribal cultures. Translators and commentators usually avoid this meaning to make poems palatable for all readers. They usually use waist or side as a substitute.
அல்குல் alkul (p. 140) – n. < id. [M. alkiṭam.] 1. Side; பக்கம். கவைத்தாம்பு தொடுத்த காழூன் றல் குல் (பெரும்பாண். 244). 2. Waist; அரை. பூந்து கில்சே ரல்குல் காமரெழில் விழலுடுத்து (திவ். பெரு மாள். 9, 7). 3. Pudendum muliebre; பெண்குறி. (திருக்கோ. 9.)
Thithilai in this context is described as beauty spots on Labia. This could be either mole or natural dark spots on vaginal area or it could be spots of oil glands known as Fordyce spots.
Yellow spots on the skin, considered beautiful in women; தேமல். பொன்னுரை கடுக்குந் திதலையர் (திருமுரு. 145). 2. Pale complexion of women after confinement; ஈன்ற பெண்களுக் குள்ளவெளுப்புநிறம். ஈன்றவ டிதலைபோல் (கலித். 32).
Per Lexicon description this could be yellow spots which look like gold sparkle as in the image depicting Fordyce spots.
